Subungual Hematoma

An injury to your finger or toenail can cause a subungual hematoma to form under the nail. These injuries are often quite minor and typically heal on their own. In the event of a more serious injury or a hematoma that is accompanied by a laceration or broken bone, medical treatment can remedy the issue and prevent further complications from occurring.

Options for Treating a Subungual Hematoma

The effects of a subungual hematoma can range from mild to severe, and minor issues involving bruising under the nail can typically be cared for at home. In this case, apply ice to the injury and take over-the-counter anti-inflammatory medication, such as ibuprofen, to help with the swelling. In the event that pain is significant, or you have a substantial injury, medical attention is recommended. There are two procedures that can help with more serious subungual hematomas:

  • Trephination – This procedure involves creating a hole in the nail to drain the hematoma and ease pressure. 
  • Nail Removal – Nail removal may be necessary if doctors must repair the tissues underneath (which is often the case with more substantial injuries).

If there are any concerns about infection, your doctor may recommend antibiotic medication. Be sure to complete the course of medication, even if the injury appears to be on the mend. You will also need to practice good wound care during the healing process, which involves cleaning the injury and changing the bandage as needed.
